Nurse Practitioner Clinic
Health for All Family Health Team is now accepting new patients.
Our Community Care Nurse Practitioner-Led Clinic provides comprehensive, team-based primary care for patients of all ages.
Your nurse practitioner will serve as your primary care provider, supporting your preventive care, acute health concerns, and chronic disease management. Depending on your needs, your care may also involve other members of our interprofessional team, including nursing, social work, pharmacy, dietetics, health promotion and care navigation.
Together, we work with you to develop a coordinated care plan, build an ongoing therapeutic relationship, and support your health and well-being over time.

What care can a Nurse Practitioner provide?
Nurse practitioners are regulated primary care providers who assess and diagnose health concerns, order and interpret tests, prescribe medications, manage acute and chronic conditions, provide preventive care, and make referrals when additional expertise is needed.
Your nurse practitioner works collaboratively with Health for All’s interprofessional team to coordinate care based on your individual needs.
How quickly can I get an appointment?
Booking your initial visit can take up to 2 weeks. However, if you need an appointment quicker, please call us, and our team will do their best to accommodate you.
Can I request a specific nurse practitioner?
Please contact our team with your request. We will do our best to accommodate preferences, including requests to join a nurse practitioner who already provides care to a member of your family. Requests are subject to provider availability and clinic capacity.

Health for All FHT’s NP Clinic is located in the Health Services building on the south side of Church Street, adjacent to the Markham Stouffville Hospital and Markham Stouffville Health Centre.
