Job Description:

Position Summary
The Medical Office Assistant is responsible for front desk operations and provides administrative support to the interdisciplinary care team, including Nurse Practitioners, Social Workers, Dietitians, Pharmacists, Health Promoters, and Management.

Key Responsibilities

  • Coordinate and book appointments for the interdisciplinary team
  • Register and intake new patients
  • Manage a multi-line telephone system, including scheduling and responding to inquiries
  • Greet patients and provide excellent customer service both in person and over the phone
  • Notify patients of appointments, recalls, referrals, and diagnostic tests as required
  • Maintain strict patient confidentiality at all times
  • Support clinical staff with basic tasks such as stocking exam rooms
  • Assist management with assigned projects and administrative tasks
  • Perform other duties as assigned

Qualifications

  • Diploma or certificate in Medical Office Administration or a related field preferred
  • Previous experience in a medical office or hospital setting is an asset
  • Strong interpersonal, communication, and organizational skills
  • Ability to multitask, prioritize, and manage a high volume of calls and patient interactions
  • Knowledge of medical terminology
  • Proficiency in Microsoft Office; experience with TELUS Practice Solutions EMR is an asset
  • Ability to work independently and as part of a team
  • Reliable attendance and punctuality
  • Flexibility to work occasional evenings and weekends as required

Job Description:

Position Summary

The Primary Care Manager provides leadership to interprofessional, physician-led teams across assigned Family Health Organizations (FHOs), programs, and services. Reporting to the Executive Director and working closely with the Physician FHT Lead, this role ensures effective team-based care, operational excellence, and alignment with HFA FHT’s strategic priorities.

This is a key leadership role focused on enabling providers to work to their full scope of practice in a collaborative and high-performing environment.

Key Responsibilities

Leadership & Operations

  • Provide day-to-day leadership, supervision, and performance management of interdisciplinary healthcare teams
  • Foster a culture of collaboration, accountability, and continuous quality improvement
  • Lead team meetings and support effective communication across providers and programs
  • Ensure continuity of clinical leadership across sites

Strategic & System Integration

  • Support implementation of HFA FHT’s strategic and operational plans
  • Collaborate with physicians, administrative teams, and partners to enhance service delivery
  • Ensure programs align with Ontario primary care priorities and evolving system needs

Quality & Compliance

  • Ensure compliance with professional standards, legislation (e.g., PHIPA, ESA, OHSA), and organizational policies
  • Lead and support Quality Improvement planning and implementation
  • Monitor service outcomes and support data-informed decision making

Human Resources & Team Development

  • Support recruitment, onboarding, and performance management in collaboration with HR
  • Promote professional development and full-scope practice across team members
  • Address team dynamics, conflict resolution, and staff engagement

Stakeholder Engagement

  • Build strong relationships with internal teams, physicians, and external partners
  • Ensure integrated, patient-centred service delivery across the community

Qualifications

  • Bachelor’s or Master’s degree in a health-related field
  • 2-4 years of leadership experience in a clinical or healthcare environment
  • Valid driver’s license and access to a vehicle
  • Successful vulnerable sector screening
  • Demonstrated experience in Human Resources is preferred; a Human Resources Professionals Association (HRPA) designation is considered an asset

About the Opportunity:

The Health for All Family Health Team (HFAHT) is seeking a passionate and skilled Clinical Pharmacist to join our growing interprofessional primary care team. We are committed to delivering comprehensive, team-based care that improves health outcomes for our diverse community.

As a Clinical Pharmacist at HFAHT, you will be embedded within family physician practices — serving as a medication expert, patient advocate, and trusted colleague. You’ll lead medication reviews, resolve drug-related problems, support evidence-informed prescribing, and make a tangible difference in patients’ lives every day.

What You’ll Do:

  • Conduct individual patient medication assessments and develop personalized care plans in collaboration with physicians and the care team.
  • Identify, prevent and resolve drug-related problems — including medication histories, monitoring, and follow-up.
  • Prescribe and adjust medications within Ontario College of Pharmacists scope of practice.
  • Utilize medical directives for expanded scope activities such as ordering labs and initiating pharmacotherapy.
  • Lead academic detailing, education sessions, and quality improvement initiatives within the primary care team.
  • Champion health promotion, chronic disease management, and evidence-informed prescribing.
  • Mentor learners and primary care colleagues on medication management best practices.

What You Bring:

Required

  • BScPhm or PharmD from a recognized university.
  • Current registration in good standing with the Ontario College of Pharmacists (Part A).
  • Completion of ACPR residency or equivalent clinical experience.
  • Demonstrated ability to work in an interprofessional team environment.

Preferred

  • 2+ years of experience in primary care.
  • Advanced credentials in pharmacotherapy, evidence-based medicine, or pharmaceutical care (e.g., BPS, CDE, TEACH, academic detailing).
  • Experience with EMRs: Telus PS Suite, Accuro, Avaros, or OSCAR.
  • Group facilitation or program planning experience.
