Case Management

Case management is the collaborative process between professional(s) and clients that involve assessment, planning, facilitation, care coordination and evaluation. It may also include advocacy for options and services to meet a client(s) and/or family’s needs.

At Health for All Family Health Team, the case manager/nurse navigator can assist with navigation and connecting to resources available in the community, navigating difficult situations that relate to your health.

Health can be affected by a broad range of personal, social, economic and environmental factors; also known as the social determinants of health. These non-medical factors can influence a person’s health outcomes. Please speak to your health care provider if you feel that you could benefit from case management or connecting to resources available within your community.
