The Health for All Family Health Team is an interprofessional primary care clinic based in Markham, Ontario. We provide a wide range of primary care services including acute, chronic and preventative health care.
Urgent Care Clinic
Our After-Hour Urgent Care Clinic is available to Health for All Family Health Team patients for booked in-person visits.
*Please do not use Urgent Care for prescription renewals, vaccinations or notes/form completion.
If you’re unable to book an Urgent Care appointment online, you can also reach us by phone at (905) 472-2200 after 4:30 pm Monday through Thursday, or after 9:00 am on Saturday.
Our Team
The Health For All FHT team is made up of doctors, family medicine residents, and allied health professionals including registered nurses, nurse practitioners, dietitians, physician assistant, pharmacist, social workers, health promoter and a case manager.
