At Health for All, the case manager/nurse navigator can assist with navigation and/or connecting to resources available in the community, navigating difficult situations that relate to your health.

Health can be affected by a broad range of personal, social, economic and environmental factors; also known as the social determinants of health. These non-medical factors can influence a person’s health outcomes. Please speak to your health care provider if you feel that you could benefit from case management or connecting to resources available within your community.

The Diabetes Program is delivered and supported by our knowledgeable interprofessional team including your family physician, family practice nurses, receptionists, pharmacist and a dietitian who is a certified Diabetes Educator.

What you can expect at your diabetes appointment:

  • Your weight and blood pressure will be taken and discussed
  • We will review your blood sugars and discuss your blood glucose monitoring 
  • We will review and explain your recent diabetes blood work results, eg attainment of targets such as HbA1C, LDL cholesterol
  • You will receive information about your diabetes, appropriate use of your medications, learn more about healthy nutrition, and exercise, and gain confidence in your ability to manage your own condition

Group Education

Group education sessions are also available in order to support healthy lifestyle changes for improved diabetes care, to understand the principles of healthy eating, to realize the importance of blood glucose monitoring, to attain targets for control and to manage medications in your diabetes care. Self Management

In order to help you with self-management of your diabetes, we encourage you to access Diabetes Canada. The information on this website can also be found in brochure format in our office. If there is a specific topic you are interested in learning more about, feel free to ask our family practice nurses, dietitian or physician at your next visit.

Health for All’s Registered Dietitian is also a Certified Diabetes Educator who can work with you and your doctor regarding:

  • Weight management
  • High cholesterol
  • High blood pressure
  • Pre-diabetes (Type 1 or 2 Diabetes)
  • Digestive disorders
  • Celiac disease
  • Prenatal or postnatal nutrition
  • Osteoporosis
  • Childhood/adolescent nutrition issues

Before your first appointment with the dietitian, record your food intake for 2 week days and 1 weekend day including:

  • What you ate and drank and amounts of each
  • Time of day

At your first appointment you will need to bring any nutritional supplements you are taking, such as vitamins, minerals, herbs and medications, along with your food record as above. Our dietitian will work with you to obtain a detailed assessment of your usual eating patterns, food preferences, intolerance/allergies to foods, daily schedule, cooking ability, etc., to help you make goals for creating positive nutrition changes for improving your health. This session will take up to 1 hour. Follow up sessions will be scheduled as required for 30 minutes.

If you have any questions about these vaccines or the schedule, you can review the yellow Personal Immunization Record that you received at your child’s first immunization appointment. You can also contact one of our family practice nurses.

Expert social workers will provide you counselling and support that is respectful of culture, race, religion, gender and sexual orientation. We provide short term counselling to a maximum of 12 sessions. The initial session will be approximately one hour, and subsequent sessions will be forty-five minutes in length.

Our social workers are an integral part of the Health for All Team and will work with you and your health care provider. They can provide confidential counselling services for a wide range of issues, including:

  • Addictions
  • Anxiety and stress
  • Depression
  • Family and parenting Issues
  • Fears and phobias
  • Grief and loss
  • Relationship problems
  • Self esteem and personal development
  • Separation and divorce
  • Trauma and abuse
  • Work related issues

Everything that is discussed in the process, as well as any documentation, is confidential between you and the Health for All Family Health Team. No information can be released to a third party without your informed consent. In circumstances where our expertise is limited or our ability to be objective is in doubt, it is our responsibility to assist you in getting the help you require.

The Prenatal program includes:

  • Regular prenatal visits with your doctor
  • Telephone contact with one of our family practice nurses to address your questions/concerns
  • Access to our registered dietitian as necessary for patients concerned about their nutritional health in pregnancy and while breastfeeding
  • Assistance with breast feeding techniques offered by our family practice nurses.
  • Access to information on prenatal classes, parenting classes, car seat safety, etc.

Booking Prenatal Care Appointments

If you have completed a home pregnancy test and have confirmation that you are pregnant, you can call us at 905-472-2200 to book an appointment with your physician. Your doctor will ask you for a urine sample and may suggest blood work be taken. You will then schedule your first prenatal visit at Health for All.

Markham Stouffville Hospital is a wonderful facility to deliver your baby. You can register with the hospital when you are 20 weeks pregnant. For more information, visit The Stollery Family Centre for Childbirth & Children.

Once you have delivered your newborn, we would be happy to care for your child at regularly scheduled Well Baby visits. Well Baby visits are scheduled regularly during the first two years of your child’s life due to the rapid growth and change that occurs during infancy. During each visit your physician monitors, advises, and answers questions on the baby’s growth and development.

Health For All FHT’s Primary Health Care Sexual Health Clinic offers comprehensive services for patients in Eastern York Region. Our clinic provides cervical cancer screening for individuals aged 25–69 (in line with Ontario guidelines), with the referring provider responsible for any follow‐up care based on exam and PAP results.

We also offer a specialized Pessary Program for those experiencing vaginal prolapse (cystocele/rectocele/apical) or stress incontinence. This program includes an initial consultation, pessary fitting, and follow-up appointments every 3–6 months conducted by our Nurse Practitioner. A $95 fee covers the cost of the pessary, and patients are encouraged to learn self-care techniques.

Additionally, our IUD Program involves 2–3 appointments, starting with counseling and prescription, followed by insertion, and a 6‑week follow-up if needed (determined during the counseling session).

In partnership with the Centre for Addiction and Mental Health (CAMH) and the Ottawa Model Smoking Cessation (OMSC) Programs, our clinic offers free nicotine replacement therapy to those who wish to participate in the STOP (Smoking Treatment for Ontario Patients) programs.

What you can expect at your Smoking Cessation appointment

Initial visit (approximately 45–60 mins):

  • Assess your readiness to quit smoking
  • Discuss the risks of smoking and benefits of quitting
  • Identify fears and barriers related to quitting, and motivational factors that will enhance your success
  • Explain the STOP program and complete the consent form if you are interested in enrolling in the program
  • Complete a brief questionnaire to review smoking habits and past quit experiences, and to identify triggers
  • Collaboratively come up with a quit date and quit plan

Follow-up visits (approximately 15–30 mins, as required):

  • Discuss quit status and smoking cessation aids if applicable
  • Discuss triggers and behavioural strategies to overcome them
  • Plan for high-risk situations
