Mental Health Program
Professional Social Work and Mental Health Counselling is available to patients by referral from your regular FHT health care provider.
Expert social workers will provide you counselling and support that is respectful of culture, race, religion, gender and sexual orientation. We provide short term counselling to a maximum of 12 sessions. The initial session will be approximately one (1) hour and subsequent sessions will be forty-five (45) minutes in length.
Our social workers are an integral part of the Health for All Team and will work with you and your health care provider. They can provide confidential counselling services for a wide range of issues, including:
- Addictions
- Anxiety and Stress
- Depression
- Family and Parenting Issues
- Fears and Phobias
- Grief and Loss
- Relationship Problems
- Self-esteem and Personal Development
- Separation and Divorce
- Trauma and Abuse
- Work Related Issues
Everything that is discussed in the process and the documentation is confidential between you and the Health for All Family Health Team. No information can be released to a third party without your informed consent.
We do not provide emergency services. In the case of an emergency please contact the emergency department of your local hospital, call 911 or call one of the crisis lines listed in the front pages of your local phone directory.
The services we offer are made available to you on a strictly voluntary basis. This means your involvement is within your discretion.
In circumstances where our expertise is limited or ability to be objective is in doubt it will be our responsibility to assist you in getting the help you require.
We request that you notify us at least twenty-four (24) hours (1 business day) in advance of your scheduled appointment if you are unable to attend, except in those circumstances that would preclude the possibility of providing advanced warning. If proper notification is not provided, this will result in the loss of that appointment as part of your session allotment.
