We are currently accepting new patients at Health For All. You can become a new patient in our Family Health Team if you do not already have a family physician, if your current physician has moved to another community, if you have moved into the same community as our Family Health Team, or if your current physician has retired.
At Health for All you have the choice of becoming a patient of our Nurse Practitioner or one of our residents which has the added benefit of longer appointments and continued access to a faculty physician.
When you require an appointment, please call us at (905) 472-2200 and press 2 to reach our admin staff who will book you the next available appointment. If your physician is unavailable there may be an opportunity for you to be seen by a Resident Physician.
If you need to cancel an appointment, please do so at least 24 hours prior to the original appointment date and time and be sure to indicate the physician you were scheduled to see, as well as the date and time of the appointment.
During your first ‘meet and greet’ visit you will be asked to provide your Health Card and Roster (enroll) with us. You will fill out paperwork that will match you to one of our physicians. This paperwork states that while you are rostered within our Family Health Team, you will not use walk-in clinics or go to other family doctors without letting us know. By committing yourself to our care, we can keep track of all of your health care needs. We will keep your medical records and personal information confidential, except as required by law.
Physicians may choose to discontinue acting as a patient’s family doctor in accordance with the College of Physicians and Surgeons of Ontario guidelines.
